Your First Visit

Please arrive 5 minutes early to check-in, with paperwork filled-out prior to arrival. 
The Initial Evaluation will usually last about 1 hour. Follow-up appointments are generally 40 minutes.

What to Bring

Loose comfortable clothes (T-shirt or tank top, sports bra, shorts, best pair of running shoes and your typical daily footwear).
We have places for you to change and store your everyday clothes or bags.


Insurance

Alpine Physical Therapy accepts most major health insurance plans including the following: Premera, Regence, Blue Cross Blue ShieldAetna, Cigna, Kaiser Permanente (PPO), First Choice, VALabor & Industries, Meritain Aetna, HMA, Lifewise, Tricare, Workers Compensation, Medicare and Medicare replacement plan (terminating 12/31/2024), and all Auto/PIP insurance.

Please contact us and we will provide a detailed insurance verification and help explain your physical therapy benefits. Depending upon your insurance plan, you may or may not need a referral.
